For new clients, an intake form is required by your insurance company and our health professionals before your appointment.
After you create your account, our system will automatically send you an email with instructions to setup your account & complete your online intake form, you can also upload your insurance information there.
Please check your email and complete the form as soon as possible to ensure smooth direct billing and avoid delays.

If you prefer a paper form, it is available in the clinic; however, please note that your treatment time may be shortened to allow time for completion.
